You can read and fill in the consent and patient history forms calmly at home — that saves time on the examination day and lets us start straight away with the important conversation.

Please note: the forms are currently available in German only. If you would like to go through them together with us in English before the examination, please bring along an accompanying person who speaks German, or come a little earlier — we are happy to walk you through them on site.
